🧪 Why "fragrance" is really a mix of unknowns

The word "fragrance" on an ingredients label can legally represent dozens of individual chemical compounds bundled together as one trade-secret-protected term — you have no way of knowing exactly what's in it. For someone with MCAS, this is a real problem: it's nearly impossible to identify which specific compound is triggering a reaction when the label gives you nothing to go on. This is part of why fragrance is such a uniquely frustrating category of trigger to manage compared to something like a specific food.

🏷️ Why "natural" and "clean" labels don't guarantee safety

Essential oils and other "natural" fragrance sources are still concentrated chemical compounds capable of triggering mast cells — being plant-derived doesn't make something inherently non-reactive. Marketing terms like "clean," "natural," or "non-toxic" are not regulated in a way that guarantees a product won't trigger MCAS symptoms. The only labels that actually mean something specific are "fragrance-free" (no fragrance ingredients added at all) — "unscented" can still contain masking fragrances added to cancel out other smells.

🚩 Where fragrance hides

  • Laundry products — detergent, fabric softener, and dryer sheets are a major, often-overlooked source since they leave residue on everything you wear.

  • Personal care products — shampoo, lotion, deodorant, and skincare frequently contain fragrance even when unlabeled clearly.

  • Cleaning products and air fresheners, including plug-ins, candles, and sprays.

  • Other people's products — perfume, cologne, or scented lotion on coworkers, family, or in shared spaces, which you can't control directly.

✅ Building a lower-fragrance environment

  • Switch to genuinely fragrance-free laundry products first — this tends to have the biggest impact since it affects everything touching your skin all day.

  • Read ingredient lists, not just front-of-package marketing, looking specifically for "fragrance" or "parfum" in the list.

  • Ask household members to switch products where possible, since your own fragrance-free choices won't help if shared spaces still carry triggers.

  • Have a plan for unavoidable exposure — discuss with your allergist what's reasonable to carry or take if you know you'll be somewhere with fragrance you can't control, like a workplace.

  • Ventilate rather than mask. Opening windows helps; scented air fresheners marketed to "purify" the air generally add another trigger rather than removing one.
